  • W-9. This form is neccessary to process Housing Assitance Payments (HAP) and is required by the IRS.
  • Direct Deposit Form. Use this form to set up electronic payments of Housing Assistance Payments (HAP).
  • Change of Ownership Form. Use this form if you are selling or purchasing a unit that is occupied by one of our families, or if you are changing where payments should be sent, or if the address where payment is sent needs to be updated.
